How to Make an Advance Medical Directive (AMD)

  • Get the AMD Form Obtain the form from clinics, polyclinics, hospitals, or download it online. Must be printed double-sided on a single sheet. 

  • Consult a Doctor and Arrange Witnesses 

    • The AMD must be signed in the presence of a doctor and one other witness (21+ years old). 

    • Witnesses must not be family members, beneficiaries, or have a financial interest in your estate. 

    • The doctor ensures you are making the directive voluntarily, with full understanding. 

    • If a doctor objects, you may approach another doctor. 

    • Submit the Form Mail or hand-deliver the completed form in a sealed envelope to 
      Registrar of Advance Medical Directives, Ministry of Health,
      16 College Road, Singapore 169854 

    • The AMD is valid only after official registration. An acknowledgement will be sent upon successful registration.
