An Advance Medical Directive (AMD) is a legal document that allows a person to voluntarily state their wishes in advance, should they become terminally ill and unconscious, to not have extraordinary life-sustaining treatment administered



How to Make an Advance Medical Directive (AMD)
Consult a Doctor and Arrange Witnesses

The AMD must be signed in the presence of a doctor and one other witness (21+ years old).

Witnesses must not be family members, beneficiaries, or have a financial interest in your estate.

The doctor ensures you are making the directive voluntarily, with full understanding.

If a doctor objects, you may approach another doctor.

Submit the Form Mail or hand-deliver the completed form in a sealed envelope to
Registrar of Advance Medical Directives, Ministry of Health,
16 College Road, Singapore 169854 -
The AMD is valid only after official registration. An acknowledgement will be sent upon successful registration.
