Medical House Call

At District mHealth, our doctor house calls offer the same level of care you would receive at our clinic — right in the comfort of your home. During the visit, our doctor will first check your vital signs, ask about your symptoms, perform a physical examination, discuss possible diagnoses and follow-up tests, and provide a prescription, with medications conveniently delivered to your doorstep.

Following the visit, if appropriate for your condition, our assigned case nurse and pharmacist may reach out to provide continued support and help enhance your care in a comprehensive, holistic way

Home-based Severe Disability Assessment

The District mHealth Team is AIC-approved to conduct Severe Disability Assessments (SDA). 

The purpose of a severe disability assessment is to ascertain suitability of a person’s medical/ mental condition to qualify for a care subsidies or withdrawals under the following severe disability schemes

  • Interim Disability Assistance Programme for the Elderly (IDAPE) 

  • ElderFund 

  • ElderShield 

  • CareShield Life 

  • MediSave Care 

  • Supplements by private insurers

To be eligible for these subsidies or payouts, a person must be assessed as severely disabled in at least three out of the six Activities of Daily Living (ADLs).

Childhood Immunizations and Developmental Assessments

Protect your child against preventable diseases with mandated and nationally recommended vaccinations by Ministry of Health through the National Childhood Immunization Schedule (NCIS). Do it in the comfort of a familiar environment! 

Childhood immunizations are fully subsidized for Singapore Citizens, at home or at any of the District Family Clinics in Singapore.

Home visits for these vaccinations are inclusive of Child Development Assessment by a doctor except for the 6th and 15th month milestone which will be attended to by a nurse if development has been progressing as expected. Standard fees will apply for Singapore PRs, long-term residents and foreigners

Certificate of Cause of Death (CCOD)

Witnessing and realizing the passing of a loved one can be stressful and challenging, especially in administrative coordination. 
 

A Certificate of Cause of Death (CCOD) is an official document issued by a licensed doctor to confirm that a person has passed away due to known and natural causes. These diagnoses include known cancer, recent hospitalizations for aspiration pneumonia or severe infections or sudden natural events like a heart attack.
 

A CCOD is required to engage a funeral director, make further arrangements for cremation of burial and place an obituary in the newspaper. 
 

At District mHealth, we can assist families to issue a CCOD if the individual was a patient at any of the District Family Clinics, or if there is a recent memo/ medical report stating the illness(s) he/she has been diagnosed with
